        The following are New York State Department of Health requirements.

 

 
 

        All new students, especially kindergartners, are required to provide proof of immunization by the
        first day of school.

        These are the required immunizations:

  • 3 DPT (triple vaccine)
  • 3 Oral Polio
  • 2 MMR Shots
  • 3 Hepatitis B

        We will need a record with Doctor's signature on it. You may contact your doctor and have him mail
        or fax a copy to us. Our fax number is: (518) 270-1659. Immunizations record must be received by 9-30-11.

        Students entering 6th grade and who are 11 years of age or older must receive an immunization containing
        tetanus toxoids, diphtheria, and acellular pertussis.

        If a student received Td, Dt, or Dtap vaccination within the last 2 years, the student's Tdap vaccination should
        be deferred until a period of 2 years has passed.

        10 year old students who are entering 6th grade will not be required to receive Tdap vaccine, but will
        need it when they turn 11.

 

 
 

        Physicals are required for all new students, Kindergarten, 1st, 3rd, 7th, and 10th graders. This must
        be obtained by your own physician, and dated between June 1st and the Opening Day of school. If you have
        any questions, please call the school office at (518) 271-0526.

        All physicals must be received in the school office by 9-30-11 and dated not more than 12 months prior to 9-10-11.

        All physicals must include visual, hearing, scoliosis screening, height, weight, and blood pressure.

 

 
 

        In order for the school to administer medication in school, we must have the following two items brought to
        the nurse's station:

  • A signed permission slip from the parent.
  • Original container from pharmacy with clear instructions, and only the amount to be taken at school in the original container.
